壓力性尿失禁

近年時常聽到病人或朋友提到尿失禁,總覺束手無策,尤其是女性,人到中年,或生育過多,或產後、久病體虛,或停經婦女,每因噴嚏、咳嗽、大笑,而出現輕微尿失禁,甚至跑步或跳動也會忍不住遺尿,實在難為情,同時亦帶來生活上的不便,不敢外遊,患者有口難言,情緒大受困擾。

現代醫學認為「壓力性失禁」,即患者因為生產,或錯誤和過量作下腹受壓運動,損害骨盆底肌肉,造成鬆弛,尿道結構容易受壓,導致用力咳嗽、劇烈運動時,腹腔壓力增加而造成遺尿,據近年醫學調查所得,壓力性失禁有年輕化趨勢,所以必須正視。

中醫認為遺尿的病因主要在於脾腎虧虛。脾虛則中氣下陷,宗筋鬆弛,膀胱容易下垂;腎虛則下元不固,膀胱收攝失調,導致小便失禁。治法以健脾益腎,收斂固澀為主。臨床常以補中益氣湯、縮泉丸或腎氣丸等加減治療,此外,配合針灸亦可提高治療效果。配合提肛運動或腹式呼吸,有助升提中氣及訓練骨盆底肌,加強骨盆底肌的支撐功能,防治尿失禁。患者應減少咖啡、濃茶、寒涼利尿之品。
